Co-Payments

If your insurance company requires you to make a co-payment, this payment is due at the time of your office visit.

Appointments

We respect your time and do our very best to honor our appointment schedule. However, due to the unpredictable nature of our specialty, occasional disruptions do occur. Just as we strive to adhere to our appointment schedule, we ask our patients to do the same. If you need to reschedule or verify an appointment, please call us at your earliest opportunity. In consideration of other patients, we may need to reschedule your appointment if you are more than 10 minutes late.

Insurance

Although our office makes every attempt to know about the general policies for a number of insurance companies, it is impossible for us to know about each individual plan. It is therefore your responsibility to know your policy and the rules upheld by your health insurance company, including covered and non-covered services.

Billing and Insurance Information

To better serve you, our billing staff will bill any services performed by our practice to your insurance company. It is imperative that we have the correct insurance information for you during each of your visits, so that we may bill the correct insurance company.

We will help you in any way we can to work with your insurance company to achieve maximum benefits; however, it is ultimately your responsibility to know your health insurance policies and the benefits covered under your plan. While our billing staff can submit your visit to your insurance company (under most circumstances you will not need to submit your own claim form), only your insurance company can determine which services are covered benefits under your plan.

Our practice accepts most insurance plans; the most accurate way to ensure that we accept your insurance company is to call your insurance company (the phone number is usually listed on your insurance card).
