Existing Patients Obstetrics Appointment
Congratulations on your new pregnancy! Below is a link to a packet of five papers for you to fill out and bring to your appointment.
The first paper is a copy of the pregnancy lab work that you will be obtaining throughout the course of your pregnancy. This is for your information.
The second paper is an informational packet. You need to read and sign the part about HIV/AIDS. The last page of that packet is a family history-type questionnaire. Please fill this out and being it with you to your first appointment with Dr. Beckman or Dr. Hopf.
The third paper is an HIV consent. This needs to be signed in order to obtain HIV testing, which is recommended by Dr. Beckman and Dr. Hopf. You will need to sign this paper and bring it with you to your first appointment.
The fourth paper is the lab order to get your prenatal blood work drawn. Please complete the name, date of birth, and check the appropriate ordering physician and take this paper to Memorial Hospital to have this drawn. We ask that you get this drawn at least a couple of days before your first appointment so that we have the results back before your appointment.
The fifth paper is the New Patient Questionnaire. Please complete and bring to your appointment.
An ultrasound will most likely be performed at your first visit. A DVD will be provided to record the ultrasound.
